I clearly see a small abrasion near the eye. Along with heavy bruising that can all cause infection in the eye which can lead to the brain.

Damian couldn't begin to understand fully understand people, but he did understand how emotions and making bad choices when your angry can make you do illogical things. So out of all the bat clan, Todd was probably the easiest to read for him.
